Terms and Conditions
By accessing or using the services of BioClarity Diagnostics, you agree to the following terms and conditions. These terms outline the responsibilities, limitations, and expectations for both the client and BioClarity Diagnostics. Clients are encouraged to review these terms before booking or participating in any service.
BioClarity Diagnostics provides mobile diagnostic services including DNA testing, drug testing, hair follicle testing, background checks, and DOT collections. All collections are performed by trained professionals under strict identity verification and documentation procedures. Services are offered on a mobile basis, and appointments are conducted at the client’s location or another mutually agreed-upon site that meets collection requirements.
Appointments may be scheduled via phone, email, or our official website. Payment in full is required at the time of booking to secure your appointment and reserve collection materials, supplies, and collector time. Quotes are based on information provided at booking; additional charges may apply for extended travel, waiting time beyond 15 minutes, parking or toll fees, or on-site complications caused by the client.
All individuals being tested must present valid government-issued photo identification at the time of collection. For legal DNA tests, the chain-of-custody documentation must be signed and verified by all applicable parties or guardians. Minors must be accompanied by a parent or legal guardian with proof of legal authority. BioClarity Diagnostics reserves the right to decline or reschedule services if identification or documentation requirements are not met.
Mobile services within a 20-mile round trip from our Brentwood, California office are included in standard pricing. Mileage beyond that distance is billed at $0.67 per mile using the shortest route listed on Google Maps. Clients are responsible for providing accurate addresses and safe, accessible testing environments. If the collector cannot safely access or perform testing due to site conditions, the appointment may be canceled or rescheduled at the client’s expense.
All testing is conducted according to company procedures and laboratory requirements. DNA collections are non-invasive buccal swabs unless otherwise specified. Drug and DOT collections follow federal and laboratory guidelines, including specimen integrity and chain-of-custody standards. Hair follicle tests require appropriate sample collection and will be submitted to accredited laboratories. Background checks are processed through authorized third-party vendors once client consent is obtained.
In the event of inconclusive or invalid results, BioClarity Diagnostics will offer a complimentary retest only when the issue is due to a company or laboratory error. If the result was compromised due to client actions, such as eating, smoking, or tampering with the sample before testing, a new test and fee will apply. Retests must be requested within 10 business days of notification.
Results are released only to the authorized individuals listed on the chain-of-custody or consent forms. For privacy and security, BioClarity Diagnostics does not provide verbal results by phone. Results are delivered through encrypted email or secure portal access in compliance with applicable privacy laws. All data is handled according to HIPAA and state confidentiality standards.
All payments are non-refundable once an appointment is scheduled or a kit has been issued. Because each test involves custom preparation, laboratory processing, and collector scheduling, refunds are not available for missed appointments, client delays, or cancellations made after collection. If BioClarity Diagnostics is unable to perform a service due to company or laboratory error, the client will receive a full refund or reschedule option at the company’s discretion.
Cancellations or reschedules must be made at least 24 hours before the scheduled appointment. Appointments canceled with less than 24 hours’ notice are subject to a $50 late cancellation fee. No-shows or same-day cancellations result in forfeiture of the full service fee. Rescheduling is subject to collector availability and must be confirmed in writing via email or text message.
Clients are responsible for ensuring that all parties are present, prepared, and cooperative at the time of collection. Failure to comply with pre-collection instructions—such as eating or drinking before a DNA test or not preparing the restroom for DOT collections—may delay testing or result in rescheduling fees.
BioClarity Diagnostics follows all applicable federal, state, and DOT regulations. The company partners only with accredited laboratories and authorized vendors to ensure test integrity and defensibility. Staff are not authorized to interpret results, provide legal or medical advice, or alter any reports issued by the lab.
By purchasing or booking a service, the client acknowledges understanding and acceptance of these terms. BioClarity Diagnostics reserves the right to update or modify these terms at any time without prior notice. Continued use of our services after such changes constitutes acceptance of the revised terms.
